WELLNESS PROGRAM ADMINISTRATOR

GENERAL SUMMARY: The Jacksonville Port Authority (JAXPORT) is seeking a qualified and licensed firm to serve as the WELLNESS PROGRAM ADMINISTRATOR who will be the designated Prime Bidder and the single point for approximately 150 full-time employees. The successful vendor shall demonstrate the capability to develop, coordinate, and administer a comprehensive employee wellness program that supports a culture of health and well-being. Services shall include, but are not limited to, develop, coordinate, and administer a comprehensive wellness program, provide and maintain an online wellness portal, coordinate wellness events, activities, challenges, and initiative programs, provide health coaching and educational programs, coordinate the annual health fair, and track and report participation in compliance with HIPAA, PHI, and applicable privacy and data security requirements. JAXPORT intends to award a contract to the responsive, responsible bidder offering the lowest fixed monthly fee consistent with meeting all specifications, terms and conditions set forth in this RFQ. To access all areas of the Port, the Transportation Worker Identification Credential (TWIC) badges are required for all Prime Contractor/Sub-Contractor employees working on the job site for this contract. Therefore, the purpose of this Request for Quote (RFQ) is to request information about your abilities to provide these services exactly as specified and to obtain a pricing commitment.
