Effective December 15, 2025, DHHS has amended solicitation #DHHS91172. The following changes have been made: - The Scope of Work has been amended. You can view the new Scope of Work, along with a redlined version posted on the procurement page here: https://dhhs.utah.gov/dhhspurchasing/dhhs91172/. - The Attachment D Service Application Form and Score Sheet have been amended with updated qualification options for Supported Employment service categories. Those documents can be found at the link above as well. These changes are reflected in the Scope of Work.
